Southern Alberta
A group of family physicians and midwives offering prenatal, birth, and postpartum care, with deliveries on the fifth floor of the Foothills Medical Centre. Includes a postpartum home-visit program, mental-health supports, lactation support, and contraceptive services (IUD/Nexplanon). Collaborates with affiliate clinic Ensemble Healthcare.

Central Alberta
A group of 14 female family physicians providing comprehensive, evidence-based, low-risk pregnancy and delivery care. Prenatal care is offered at the Allin Clinic, with deliveries at the Lois Hole Hospital for Women (Royal Alexandra Hospital) and a physician on call every day, including weekends and holidays. Postpartum care continues to six weeks.
A family-physician group based in Canmore with a satellite clinic in Cochrane, providing low-risk obstetrical care (preconception counselling, prenatal, postpartum, bedside ultrasound). Offers shared care and C-sections as needed, and can arrange shared care for patients delivering in Calgary. Accepts self- and physician referrals.
Ten experienced family physicians plus a labour-and-delivery RN, seeing patients from as early as 10 weeks at the SASPCN shared-care clinic, with deliveries at the Sturgeon Community Hospital. An official University of Alberta teaching site (residents and students may participate). Postpartum visits at one and four weeks.
A team of experienced family physicians, partnered with the Edmonton West Primary Care Network, providing prenatal and postnatal care to low-risk patients with 24-hour on-call service and deliveries at the Misericordia Community Hospital. Appointments are coordinated so patients meet all clinic physicians before delivery. Open to all patients; no referral required.

Greater Toronto Area (GTA) & Central
Credit Valley Family Health Team provides pregnancy care through a collaborative team of family doctors, residents and physicians trained in obstetrics. Patients are connected with an obstetrical practice at Credit Valley Hospital, while their family doctor remains involved throughout pregnancy and provides newborn follow-up after birth.
The Family Way Obstetrical Group at Women's College Hospital provides family physician-led care throughout pregnancy, birth, the postpartum period, and newborn care. Patients are supported by an interdisciplinary team, including nurses, dietitians, social workers, lactation consultants, pharmacists, physiotherapists, and occupational therapists.
Michael Garron Hospital’s Family Medicine Obstetrics provides comprehensive pregnancy, labour and delivery, and postpartum and newborn care. Patient-centred prenatal care is conveniently offered at various clinics across East York, and deliveries occur at Michael Garron Hospital. Patients can generally be accepted at any stage in pregnancy with flexible referral pathways including self referral.
The Mount Sinai Academic Family Health Team provides comprehensive, evidence-based care throughout pregnancy, birth, and the postpartum period. Their interdisciplinary team of family physicians, nurses, dietitians, pharmacists, social workers, and midwives work together to deliver compassionate, patient-centred care for growing families.
North York General Hospital’s Family Medicine Obstetrics team provides comprehensive pregnancy, labour and delivery, postpartum and newborn care. Their family physicians offer personalized, team-based care and work closely with obstetricians and other specialists when additional support is needed.
Patients can access the program even if they already have a family doctor, and self-referrals are accepted. After pregnancy and postpartum care are complete, patients and their babies can return to their regular primary care provider for ongoing care.
St. Joe’s Toronto Family Medicine Obstetrics provides pregnancy, labour and delivery, postpartum and newborn care through a team of family physicians. Prenatal care is offered at community clinics across west Toronto and Etobicoke, with deliveries at St. Joseph’s Health Centre. Self-referrals are accepted.
The Obstetrical Care Program at the Toronto Western Family Health Team provides family physician-led care throughout pregnancy, birth, and the postpartum period. Patients receive coordinated, team-based care from family physicians, nurses, social workers, and family medicine residents, with access to Mount Sinai Hospital specialists when additional support is needed.
North
The Thunder Bay Family Medicine Obstetrics team provide comprehensive pregnancy care throughout the prenatal period, birth, and for up to two months postpartum for both mother and baby. We are happy to see you from the very beginning of your pregnancy, or accept a referral for care in the later stages of pregnancy from family doctors or nurse practitioners who provide early prenatal care.
