Includes a postpartum home-visit program, mental-health supports, lactation support, and contraceptive services (IUD/Nexplanon). Collaborates with affiliate clinic Ensemble Healthcare.

Patients are supported by an interdisciplinary team, including nurses, dietitians, social workers, lactation consultants, pharmacists, physiotherapists, and occupational therapists.
Michael Garron Hospital’s Family Medicine Obstetrics provides comprehensive pregnancy, labour and delivery, and postpartum and newborn care. Patient-centred prenatal care is conveniently offered at various clinics across East York, and deliveries occur at Michael Garron Hospital. Patients can generally be accepted at any stage in pregnancy with flexible referral pathways including self referral.
The Mount Sinai Academic Family Health Team provides comprehensive, evidence-based care throughout pregnancy, birth, and the postpartum period. Their interdisciplinary team of family physicians, nurses, dietitians, pharmacists, social workers, and midwives work together to deliver compassionate, patient-centred care for growing families.
North York General Hospital’s Family Medicine Obstetrics team provides comprehensive pregnancy, labour and delivery, postpartum and newborn care. Their family physicians offer personalized, team-based care and work closely with obstetricians and other specialists when additional support is needed.
Patients can access the program even if they already have a family doctor, and self-referrals are accepted. After pregnancy and postpartum care are complete, patients and their babies can return to their regular primary care provider for ongoing care.
St. Joe’s Toronto Family Medicine Obstetrics provides pregnancy, labour and delivery, postpartum and newborn care through a team of family physicians. Prenatal care is offered at community clinics across west Toronto and Etobicoke, with deliveries at St. Joseph’s Health Centre. Self-referrals are accepted.
The Thunder Bay Family Medicine Obstetrics team provide comprehensive pregnancy care throughout the prenatal period, birth, and for up to two months postpartum for both mother and baby. We are happy to see you from the very beginning of your pregnancy, or accept a referral for care in the later stages of pregnancy from family doctors or nurse practitioners who provide early prenatal care.
